Oh come on man. The context that it was used, in each circumstance was different. McCain was calling Hillary's healthcare program the same pig proposed in the past with lipstick - a reference to a program. Obama was referring to the McCain (the "more of the same" candidate) campaign that just chose a woman as the veep. He added lipstick to the same old pig. There is a difference in the way they used it.
someone call in to talk radio and explain that the REAL expression is actually:
"You can put perfume on a Pig
But it still smells like a Pig"
I checked it on Google and apparently the caller was correct.
Which means by switching it to 'Lipstick' it was definitely supposed to insult Sarah Palin who said in her speech, "The difference between a Hockey Mom and a Pitbull is Lipstick"

We all no dam well that Obama had an underlying agenda to that remark. He is smart enough to know that if he made such a comment, it would be taken that way. Therefore, you think he would have not said it. The point is that he did say it and many people will take it that way no matter how he tries to explain himself. He really screwed himself on that one. The Dems must have really done some digging to come up with that one .
