To increase appetite I'd go with the B-12 over the EQ @ 200mg/wk. That's a really low dosage (400mg/wk is almost universally considered the minimum dose).
Since you're only looking for around ten lbs (according to your original post), the test and deca should really be all you need. Next time around try running the test/deca/eq if you so desire. Just my 2¢.
As for the dbol, since it's your first time, 30mg/day for the first few weeks should kickstart your cycle nicely. You can always up the dose a bit if you deem it necessary.
