That does not look real. It appears that we are looking at his brain, but they would have to cut away the cranium to do that. Looks like some photoshop action, or some creative special effects makeup. -
I agree...not real - do you see the large red "mass" potruding from his forehead? THAT would be cause for concern WAAAAY before it got that big.
