How well are you warming up when you do bench?
Do you progress often of stay at the same weights for long stretches?
Do you ever go real heavy for just 3 or so?
I don't really do warmups because it seems to lower the amount of weight I can handle. Unless I'm trying for my 1 rep max which I'll put up 115lbs like 10 times to warmup, then I'll go for my max. But for regular workouts I never warmup.

I can not push as much weight if I DON'T warm up. My warm up is something like:
135 x 5 reps
225 x 3 reps
315 x 1 rep
Then I do max or the appropriate weight for doubles or triples. I use this approach with all my heavy lifts (deads, squats, etc...)
if there is any exercise i NEED to warm up on it's flat bench. i always do 2 sets of 135 for 15 and then a set of 185 for 12... i also do shoulder rotations for my rotator cuffs with a 5lb. plate in between those sets combined with LOTS of stretching. the more fluid i get going the stronger i am, plus i am greatly reducing my chance of injury, which is pretty high on flat bench if am cold.
especially when i am "on", and i have fluid in my joints... it's too easy to not warm up because my joints feel so good.
