Tweetfast food and plenty of small restaurant chains are just as bad. The owners do anything to not overhaul the kitchen and they get nasty! like stated above employees are the majority of the problem

taquerias and mom and pop Chinese restaurants seem to score lowest.

it is a cultural thing. in some countries its fine to store bread on the bottom shelf with raw chicken thawing above it.
