Abstract
CONTEXT:
Testosterone (T) is commonly administered intramuscularly to treat hypogonadal males and female-to-male (FTM) transgender patients. However, these injections can involve significant discomfort and may require arrangements for administration by others.
OBJECTIVE:
We assessed whether T could be administered effectively and safely subcutaneously as an alternative to intramuscular (IM) injections.
DESIGN:
Retrospective cohort study.
SETTING:
Outpatient reproductive endocrinology clinic at an academic medical center.
PATIENTS:
Sixty-three FTM transgender patients aged >18 years electing to receive subcutaneous (SC) T therapy for sex transition were included. Fifty-three patients were premenopausal.
INTERVENTION:
Patients were administered T cypionate or enanthate weekly at an initial dose of 50 mg. Dose was adjusted if needed to achieve serum total T levels within the normal male range.
MAIN OUTCOME MEASUREMENTS:
Serum concentrations of free and total T and total estradiol (E2), masculinization, and surveillance for reactions at injection sites.
RESULTS:
Serum T levels within the normal male range were achieved in all 63 patients with doses of 50 to 150 mg (median, 75/80 mg). Therapy was effective across a wide range of body mass index (19.0 to 49.9 kg/m2). Minor and transient local reactions were reported in 9 out of 63 patients. Among 53 premenopausal patients, 51 achieved amenorrhea and 35 achieved serum E2 concentrations <50 pg/mL. Twenty-two patients were originally receiving IM and switched to SC therapy. All 22 had a mild (n = 2) or marked (n = 20) preference for SC injections; none preferred IM injections.
CONCLUSIONS:
Our observations indicate that SC T injections are an effective, safe, and well-accepted alternative to IM T injections.

Tweetif you think about how most sub q compounds are done they are done in water. why is that? because it will release into your system much faster. now if you go in and put oil in your fat it is going to take a hell of a minute to absorb. that leaves you with a better chance of having pain and getting an infection. if you have ever hit in between a muscle or something like that where the oil doesnt get into the muscle it is extremely painful for a while. no way in the world i would deliberately put an oil based compound in sub q for those very reasons. ouch

Tweeti always thought of it like this: why roll the dice of either getting an abcess or some serious pain for an extended period of time? if i want a product that is going to release slower or stay in my system longer i will go with test u or something similar but still i like to keep my levels as close to stable as possible and have no fluctuations going on. i wont knock anyone for doing it but to me the risk we take has to be ripe with rewards. i have had some really bad hits in my time to self inflicted so i will not be doing something i know could hurt like a mother for a long time haha

TweetI am on TRT and sub Q is all I do. no more intra muscular shots for me. way easier, no need to aspirate (although I do) painless. I just shoot into the stomach. I do not use a tuberculin syringe. I just use a regular 3 ml with a 23ga pin. much faster to fill with a 20ga pin and shoot with the 23ga.
I have not gone more than 1ml though. normally 1/2 ml for my TRT but when I switched 2 years ago my numbers did not change even one bit. So it does work.
